Hunt Country Health Services is now providing Primary Home Care, a program that brings the doctors office into your home it is aimed at illness prevention and health maintenance.
Our board certified Nurse Practitioners bring the doctor’s office to your home on a regular monthly schedule, along with sick call visits and 24 hour telephone on-call service.

From house calls by Nurse Practitioners to specialty services such as in-home laboratory blood draws, cardiology, podiatry, optometry, Ultrasound, X-Rays, physical and occupational therapy, and skilled nursing, we are able to provide the most-inclusive house call medical services available.

In addition, we can arrange and manage nursing, hospice, prescriptive medicine, durable medical equipment, and home delivery of medications and diabetic supplies.

Scheduling a Home Visit
Home visits are scheduled on a regularly monthly basis through our office. Patients are normally called 24 to 48 hours prior to arrange a visit. Unfortunately, we are unable to provide a specific time for the visit due to travel time and the needs of each patient seen that day. Follow-up visits are scheduled based upon your specific needs.

Sick Calls
If the patient becomes ill and is in need for a home visit, we will do our best to fit you into the schedule within 48 hours of your call. Primary HomeCare is not an emergency service. If you feel that your situation is urgent, or you cannot wait for a visit, please call 911.

Call Backs
Primary HomeCare provides 24 hour telephone on call coverage 7 days a week. If you have caller ID with call rejection on your phone, you will have to disable it by dialing *87. After your call back, turn the service back on by dialing *77.

Perscriptions
We are unable to provide prescription refills after hours or on weekends. If you need a prescription refilled, please call our office during regular business hours from 9AM to 5PM, Monday through Friday. Primary Home Care offices are closed on weekends and the on-call provider does not have access to your medical records.

Patient’s Responsibilities
We ask that you take your medications as prescribed and keep us informed about emergency room visits, hospitalizations and changes in your insurance or your address
